Victorians have the right to access or correct health information our organisation holds about them.
Access to health information requires proof of identity. Requests made on behalf of another person must be made in writing, and must include evidence of their authority to act on behalf of the other person.

Should you wish to make an application to access information from your medical record please complete the Request for Access to Patient Information form.
Once your application form has been received by the Privacy Officer, a search will be conducted for the requested information. Under the Health Records Act 2001, the organisation has up to 45 days to provide the information requested.
If a correction is required to your patient information, please complete the Request for Correction of Patient Information form.
Please post/fax/email the completed form to Ramsay Clinic Albert Road together with a photocopy of photo identification (e.g. your driver’s licence or passport).
