Supporting your recovery journey in the community

The Ramsay Mental Health Recovery Support Program (RMHRSP) is a personalised, recovery-focused service designed to support individuals living with mental illness. Care is delivered in the setting that best suits your needs - whether that’s in hospital, at home, or in the community.

What the Program Offers

RMHRSP provides a 12-months of individually tailored specialised community mental health support program to help you:

  • Access the services and resources you need
  • Build confidence in managing your mental health
  • Work toward your recovery goals
  • Transition safely from hospital to home
  • Strengthen your independence and self-care in the community

Who The Program Is For

This program is available to *eligible individuals with private health insurance (currently supported by, AHM, Medibank Private and BUPA) who have been admitted for an overnight admission where the treating psychiatrist, in collaboration with the patient and the clinical care team agree this model of care would be beneficial.

How It Works

Your care will be coordinated by the Recovery Care Navigators at Ramsay Clinic. (They will work closely with you, your assigned Case Manager, and other service providers to ensure your care is tailored to your needs.

Support May be Delivered:

  • Through in-home and community visits
  • In person at outpatient clinic appointments
  • Via telehealth consultations
  • As group sessions within the community or at the clinic

The frequency and location of your appointments will be based on your clinical needs and decided in collaboration with you and your treating team.

Services Available

Available services depend on clinical staff (health disciplines) availability.

Your care will be coordinated by a Case Manager with options to include a range of mental health services tailored to best support your specific needs. For example:

  • Clinical nursing
  • Psychology
  • Social Work
  • Dietetics

Where a specialised service is not available through the program you will need an external referral from your treating doctor. The costs for these external services are not funded by the Program, and you will be financially responsible for external service costs. While the Program supports this process, it is the responsibility of your GP/Psychiatrist to provide external referrals per Medicare requirements.

Day Programs

If your treating Psychiatrist refers you to one of the clinic Day Programs, as a patient enrolled in this Program you can also attend a Day Program at the same time.  

The Program focuses on these 3 outcomes for patients:

Program Aim 1: Support Recovery After Discharge

In the initial stage of the program, care is focused on meeting your immediate recovery needs following discharge. The aim is to enhance your personal, social, and occupational functioning, while supporting overall wellbeing and adjustment to life in the community.

Program Aim 2: Build Skills and Independence

The focus of Goal 2 is centred on helping you practise and apply coping strategies in real-life settings. The goal is to build confidence, increase independence, and continue strengthening your personal, social, and/or occupational functioning.

Program Aim 3: Maintain Progress and Long-term Recovery

 The focus is on maintaining the progress you've made and promoting long-term stability. This includes supporting your ability to live independently, optimising functioning, and reducing the risk of relapse or unplanned readmission to hospital.

After 12-months of successful Ramsay Mental Health Recovery Support, you will be discharged from the Program.

Your Privacy and Participation

You’ll be asked to complete some paperwork before and during the program. All personal and clinical information is handled confidentially and in line with the Australian Privacy Principles.

To help your health fund evaluate the program’s effectiveness, some administrative and clinical outcome data will be shared with them. By joining this Program, you consent to this data being provided.

Participation in the program is entirely voluntary, and you are free to withdraw at any time.  

*Medibank Private/AHM patients admitted for the treatment of an eating disorder or addiction are not currently permitted to be enrolled in the program. Patients admitted for the purposes of participating in a clinical research trial are also not able to be enrolled in the program.
