The Ramsay Mental Health Recovery Support Program is a personalised, recovery-focused service designed to support individuals living with mental illness. Care is delivered in the setting that best suits your needs - whether that’s in hospital, at home, or in the community.
The Ramsay Mental Health Recovery Support Program provides a 12 month, one-on-one support pathway to help you:
This program is available to eligible individuals with private health insurance (currently supported by Medibank Private and BUPA) who are living with a mental illness and may be at risk of hospital admission or readmission. You will also need to meet the following criteria:
Your care will be coordinated by the Recovery Care Navigators at Ramsay Clinic New Farm. They will work closely with you, your assigned Case Manager, and other service providers to ensure your care is tailored to your needs.
Support may be delivered
The frequency and location of your appointments will be based on your clinical needs and decided in collaboration with you and your treating team.
Available services depend on the clinical staff (health disciplines) available.
Your Clinical Nurse will be your primary health professional and case manager during your participation in the program responsible for coordinating appropriate care and referral to other health disciplines.
Currently Ramsay Clinic New Farm offer the following services:
If you require services currently not available at Ramsay Clinic New Farm you will need an external referral. The costs for these external services are not funded by the Program, and you will be financially responsible for external service costs. While the Program supports this process, it is the responsibility of your GP/Psychiatrist to provide external referrals per Medicare requirements.
As a patient enrolled in this Program you can also attend a Day Program at the same time. This option is not available to other patients or health fund members. Click here to view our extensive list of day programs available.
The Program is delivered over three phases
Phase One – Transition | Phase Two – Consolidation | Phase Three – Maintenance |
---|---|---|
During the first month (approximately), care is focused on supporting your immediate recovery needs following discharge. The aim is to enhance personal, social, and occupational functioning, as well as overall wellbeing. | In the following three months, care shifts to helping you practise and apply coping strategies while building independence. The goal is to continue improving your personal, social, and/or occupational functioning. | For the remainder of the program, care is centred on maintaining progress and supporting long-term stability. This phase focuses on optimising functioning, reducing the risk of relapse, and supporting you to live independently with confidence. |
After 12-months of successful Ramsay Mental Health Recovery Support, you will be discharged from the Program.
You’ll be asked to complete some paperwork before and during the program. All personal and clinical information is handled confidentially and in line with the Australian Privacy Principles.
To help your health fund evaluate the program’s effectiveness, some administrative and clinical outcome data will be shared with them. By joining this Program, you consent to this data being provided.
Participation in the program is entirely voluntary, and you are free to withdraw at any time.
