To arrange an outpatient appointment, assessment or admission a referral for your patient is required to be faxed to 02 6659 4456.

Please give as much information as possible including:

• Detailed history of patient including:
• Aggression or violence;
• Suicide attempts or self harm;
• Substance abuse; and

• Level of mobility in elderly patients.

• Private health insurance details. We can check the level of cover and advise patient of any out-of-pocket expenses
